Each year the American Association of Nurse Practitioners (AANP) recognizes supporters of nursing from across the country, in two categories: nurse practitioners who have made outstanding contributions; and advocates who have made significant contributions toward increasing the awareness and acceptance of nurse practitioners.

This year AANP gives its State Award for Excellence to a number of advocates with close ties to the Future of Nursing: Campaign for Action, an initiative of AARP Foundation, AARP, and the Robert Wood Johnson Foundation. Those allies include two state legislators who work with AARP state offices and sponsored bills to allow advanced practice registered nurses the ability to practice to their full education and training.

In the advocate category, here are the award winners who also have ties with the Campaign:

Josh Cockroft understand the immediate need and urgency of finding a solution,” said Chad Mullen, associate state director of Advocacy at AARP Oklahoma. “We appreciate his willingness to stand up against a powerful and entrenched industry in order to bring much needed access to care to Oklahomans across the state.”

In Texas, Blake Hutson, associate state director for Advocacy and Outreach at AARP Texas, said, “Rep. Klick has been a consistent leader at the Capitol on nurses’ issues and a close partner with AARP in our efforts to promote the role of Advance Practice Nurses. She’s not afraid to stand up against heavyweight interests that want to keep the status quo in medicine to the detriment of Texas patients.”

Problem Statement: The Triple Aim of Health Care focuses on improving the experience of care, improving the health of populations, and reducing per capita costs of health care. Nurses, regardless of where they practice, will play key roles in achieving the population health aim.  What is needed to develop the skills that broaden the nurse’s vision into the community?  How do we engage nurses in providing services to clients by seeing the bigger picture of how their work at the “bedside” extends into the community?

Approach: This presentation describes creating an academic-practice partnership using a community-based rotation for all nursing students. We engaged a community steering committee of state and local public health, civic, academic, and clinical nurse leaders to develop and pilot an intensive curriculum through a local hospital’s summer nurse internship program. Working in a hospital clinical area with an assigned nurse mentor, students engaged in a variety of population focused activities using community and civic engagement strategies.

Products/Outcome: Community-based curriculum tied to clinical areas and populations seen in the hospital. Pre- and post-internship data will be presented. Students improved knowledge, skills, attitudes and behaviors. Student and mentor feedback provides lessons learned for improving a community-based clinical rotation that builds a culture of health.

Implications: Academic-practice partnerships engaging nursing students in community-linked clinical rotations will help prepare future nurse leaders to foster a culture of health. Preliminary data shows an education cycle of content, practice and reflection with a focus on creating a sense of community and citizen agencies, develops more awareness around population health management and building a culture of health, regardless of the area of practice.

Does modernizing state nursing practice laws impact businesses in America?

The Bay Area Council Economic Institute set out to answer that question, and this page contains whitepapers on what they found.

“Simply put: an inefficient delivery system that unnecessarily restricts health professionals from practicing to the full extent of their training is bad for the economy and bad for business” says one of the reports, “Full Practice Authority for Nurse Practitioners Increases Access and Controls Cost.”

The Institute took a close look at California, Kentucky, Minnesota and New Jersey. It found that businesses would save significant amounts of money in each state if nursing practice policies were modernized to provide consumers more direct access to nurse practitioners.

Full Practice Authority for Nurse Practitioners Increases Access and Controls Cost

The Bay Area Council Economic Institute designed a model to measure the value that state nursing scope of practice policies have on businesses’ bottom lines. They applied that model to several states and identified that businesses would save significant money if their respective states’ nursing scope of practice policies were modernized. Below are the original model and the state reports they have produced, to date.


Full Practice Authority for Nurse Practitioners Increases Access and Controls Cost: Spotlight California

If California’s practice restrictions were lifted, there would be an additional 4,000 nurse practitioners working in the state today, an increase of 24 percent.

It is estimated that if California had full practice authority for nurse practitioners it would save $1.8 billion in the cost of preventative care visits in the first 10 years.


Full Practice Authority for Nurse Practitioners Increases Access and Controls Cost: Spotlight Kentucky

If Kentucky’s practice restrictions were lifted, there would be an additional 500 nurse practitioners working in the state today, an increase of 18 percent.

It is estimated that if Kentucky had full practice authority for nurse practitioners it would save over $200 million in the cost of preventative care visits in the first 10 years.

 Full Practice Authority for Nurse Practitioners Increases Access and Controls Cost: Spotlight Minnesota

Minnesota granted nurse practitioners full practice authority in the beginning of 2015; if the restrictions were lifted previously, there would be an additional 600 nurse practitioners working in the state today, a 20 percent increase.

It is estimated that Minnesota will save over $250 million in the cost of preventative care visits for the first ten years.


Full Practice Authority for Nurse Practitioners Increases Access and Controls Cost: Spotlight New Jersey

If New Jersey’s practice restrictions were lifted, there would be an additional 1,050 nurse practitioners working in the state today, an increase of 25 percent.

It is estimated that if New Jersey had full practice authority for nurse practitioners it would save over $400 million in the cost of preventative care visits in the first 10 years.

Minnesota Alliance for Nursing EducationIn Minnesota, hundreds of rural nursing students will soon have a more streamlined pathway to the baccalaureate degree thanks to the persistence and foresight of nursing leaders in the state. This past year, 1,500 residents applied to a new dual-admission bachelor of science in nursing (BSN) program, a collaboration between seven community colleges and Metropolitan State University that will allow students to earn a baccalaureate degree in their home communities.

The Minnesota Alliance for Nursing Education (MANE) began exploring ways to increase the number of baccalaureate-prepared nurses in the state even before the Institute of Medicine’s Future of Nursing report recommended preparing 80 percent of the nursing workforce at the baccalaureate level or higher by 2020. The report, and the subsequent formation of the Minnesota Action Coalition, gave MANE a welcome boost. The Action Coalition saw the alliance as a perfect vehicle for pursuing the Campaign’s “80 by 20” goal, and thanks to a State Implementation Program grant from the Robert Wood Johnson Foundation, the Action Coalition has provided MANE with the financial and technical resources it needs to fully develop and implement its vision of a shared, competency-based BSN curriculum.

MANE schools have now admitted their first cohort of 850 students, who will begin their nursing studies this fall.

“This gives me goose bumps,” said Lynn Johnson, MSN, RN, who has worked as director of nursing at Ridgewater College in rural Minnesota for three decades. Johnson had been looking for a way to transform nursing education at her school since 2006.

MANE’s new competency-based BSN curriculum departs dramatically from Ridgewater’s associate degree curriculum, which was built on a foundation year of practical nursing. Moving forward, the first two semesters will be devoted to general education and the next three to professional nursing to ensure students are well-prepared.

“We start with critical reasoning right off the bat,” said Johnson. “The first nursing semester, students focus on health promotion and wellness, all with well clients; the second semester is chronic and end-of-life care; and the third semester addresses acute and complex conditions.”

The final three semesters focus on baccalaureate content, including public health and global health. To promote mastery, the “spiraled” curriculum revisits most concepts each semester in a different context and at a different level of complexity.

The Action Coalition would like to see MANE expand to more community colleges and four-year institutions throughout the state, but even at its current size, MANE should have a major impact on meeting the Campaign’s80 by 20 goal. According to Faye Uppman, MS, RN, who leads the MANE project, only 35 percent of Minnesota’s nursing graduates currently earn prelicensure baccalaureate degrees.

“Associate degree programs graduate twice as many nurses,” she said, “but if even half of MANE enrollees complete the baccalaureate, we are going to shift that pendulum.”

Although students have the option of leaving the program with an associate degree after five semesters, Uppman is optimistic that most students will stay to earn a BSN.

“Our patient population needs nurses who are prepared to be thinkers in any environment, from acute care to home care to case management to telehealth nursing,” she said.

MANE’s founders also recognize the importance of increasing the diversity of the nursing workforce in Minnesota, and hope the program will help to attract more diverse students. Uppman acknowledges, however, that it is not enough simply to recruit diverse students.

“Once students are accepted, how can we best create a culture of inclusiveness and support?” she asked. That is one of the questions the Minnesota Action Coalition and MANE are working to answer.

Minnesota Removes Barriers to Full Practice Authority for APRNsMay 13, 2014, Minnesota’s Senate Bill 511 was signed into law by Governor Mark Dayton. The new law will increase consumer access to health care and reduce unnecessary health care costs by granting full practice authority to all advanced practice registered nurses (APRNs) in Minnesota. It will go into effect on January 1, 2015. However, new nurse practitioners and clinical nurse specialists will have to complete 2,080 hours of practice under a collaborative agreement with a physician or experienced APRN before achieving full practice authority, and certified registered nurse anesthetists would still need a collaborative agreement for intervention pain management procedures.

Congratulations to the many individuals, groups, and organizations that supported this important legislation, especially the members of the Minnesota Action Coalition and AARP Minnesota. With the passage of this law, Minnesota becomes the second state this year to pass legislation removing outdated barriers that prevent APRNs from providing care to consumers to the full extent of their education and training.

Today the Robert Wood Johnson Foundation (RWJF) announced that 10 states will be part of a $4.5 million initiative, the Future of Nursing State Implementation Program. The program is helping states prepare the nursing profession to address our nation’s most pressing health care challenges—access, quality, and cost. The program launched with 20 states in February.

The State Implementation Program bolsters efforts already underway in 50 states and the District of Columbia—the Future of Nursing: Campaign for Action—to improve health and health care through nursing. A joint initiative of AARP and RWJF, the Campaign is working to implement the Institute of Medicine’s (IOM) evidence-based recommendations on the future of nursing. It provides a vehicle for nurses at all levels to lead system change to improve care for patients and families through collaboration with business, consumer, and other health professional organizations.

The State Implementation Program is providing two-year grants of up to $150,000 to a total of 30 state-based Action Coalitions that have developed or made substantial progress toward implementing the IOM recommendations. States must obtain matching funds to receive the grant. Grants were announced today in: Alabama, Alaska, Arkansas, Illinois, Minnesota, Nevada, Ohio, South Carolina, Vermont, and Virginia. The 20 states that received grants in February are: Colorado, Connecticut, Florida, Georgia, Iowa, Idaho, Kansas, Louisiana, Maryland, Michigan, Missouri, Mississippi, Nebraska, New Jersey, Pennsylvania, Rhode Island, Tennessee, Utah, Wisconsin, and Wyoming.

The Center to Champion Nursing in America (CCNA), an initiative of AARP, the AARP Foundation, and the Robert Wood Johnson Foundation, serves as the national program office for the Future of Nursing State Implementation Program.
